Community health assessment meetings and follow up meetings involve a process of collecting, analyzing, and using data to educate and mobilize communities, develop priorities, garner resources, and plan actions to improve the public's health. It is one of the core functions of public health. . The meetings involve the systematic collection and analysis of data in order to provide the Office of Public Health and the community it serves with a sound basis for decision-making. The meetings are conducted in partnership with other organizations in the community and include collecting data on health status, health needs, community assets, resources, and other community or state determinants of health status.
The Office of Public Health will be using the Mobilizing for Action through Planning and Partnerships (MAPP) methodology to complete the health assessments. The MAPP process is a community-driven strategic planning process for improving community health. The meetings with be facilitated by public health leaders as this framework helps communities apply strategic thinking to prioritize public health issues and identify resources to address them. MAPP is not an agency-focused assessment process; rather, it is an interactive process that can improve the efficiency, effectiveness, and ultimately the performance of local public health systems.
