What is Transition?

A National goal for all Youth with Special Health Care Needs is to receive appropriate Transition Services. What does this mean?

Transitioning from adolescence to adulthood is a challenge for all youth, but Youth with Special Health Care Needs may require additional services to help them become independent.  These services may include the following:

1. Finding appropriate adult health care, which includes providers, and insurance plans that will cover needed services

2. Identifying work and/or school related opportunities

3. Independent living
