Managing Your Health Information

There are a variety of tracking and recordkeeping tools and websites for managing your and your loved one’s health information. It's wise to take a printed record of your current health information with you when you visit your health care provider and when you travel. An online personal health record allows you to access and update the information from anywhere. Use these resources consistently to help you and your health care team keep track of your overall health, your medications and your health care decisions. While your health care team keeps records, it’s important you do too.
