Change of Ownership - Licensing RequirementsState law requires that copies of all documents relating to the change of ownership be submitted along with a description of the change of ownership (lease, purchase of assets, etc.), the effective date and the name and address of both the old and the new owners. The change of ownership must be submitted to Health Standards Section within five (5) working days of the effective date.

Licenses are not transferable; therefore, another licensing application and fee must be submitted. The fee must be submitted in the form of a company check, certified check, or money order made payable to the Department of Health and Hospitals.

Change of Ownership Packets are available upon request. Please complete the Information/Request Order form, indicate program type, include packet fee and return to HSS.
