Sequence for Putting on PPE

To Providers

We understand that there has been significant requests for PPE and other medical supplies/equipment.  Previously you may have been told to go through your Parish EOP (Emergency Operations Center) or use the app, ReadyOps – effectively immediately, please discontinue using those avenues.

IF YOU ARE REQUESTING PPE:

In an effort to streamline that process, we have implemented the following protocols for you to seek PPE/supplies that you need.

  1. To the extent possible, please continue to use the vendor/supply chain you would normally use to purchase/procure supplies/PPE needed.
  2. We are keeping a list of unique providers (as they become known to us) listed below. You may be able to receive supplies/PPE more quickly by purchasing from them directly.
  3. You can request PPE from the State by emailing Christy.Sawyer@la.gov and Sara.Williams2@la.gov; however, this process has changed. Each entity requesting PPE from the state must provide documentation/proof that your entitle has:
  • Reached out to a minimum of five (5) vendor, in the last 7 days, for PPE (this can be from your normal supply chain and/or vendors from the LDH list). This documentation MUST be from the vendor (fax, email or documentation from the vendors website indicating the vendors:
    • Cannot provide any of the needed PPE.
    • Can only provide a partial order or needed PPE
    • Can supply but there is an unknown date of when PPE will be received/delivered

It is important to note we will NOT accept a list of vendors contacted along with their phone number.

Your email must include:

  • Agency Name
  • Contact Person and Phone Number
  • Items being requested and quantity (if you are requesting 5000 gowns and the vendor can supply you with 2500, you cannot request the full 5000 from the State. You can only request what you CANNOT receive from the vendor(s).
  • Proof that vendors cannot provide the following:
    • Cannot provide any of the needed PPE.
    • Can only provide a partial order or needed PPE
    • Can supply but there is an unknown date of when PPE will be received/delivered

Once all information has been received via email and reviewed, the State will determine if a PPE order can be accepted. This information will be communicated with you via email and/or phone.

Please remember that by submitting your email and documentation, this is not a guarantee that you will receive PPE. You may receive some or none of what you requested depending on the supply availability. We understand there continue to be shortages with some PPE items, so please be patient as we distribute our limited resources the best way we can.

IF YOU ARE NEEDNG AN UPDATE TO AN ORDER

Please email Christy Sawyer at Christy.Sawyer@la.gov  to obtain a status of your order. You will need to include the following information in your email:

  • Agency name
  • Agency contact name and phone number
  • Mission number provided at the time order was placed
  • Approximate date (at least month) order was placed
  • Parish of agency
