Notification of Pregnancy Form Now Available

The Notification of Pregnancy (NOP) form is now available for Medicaid providers on the Healthcare Provider's page at www.makingmedicaidbetter.com under "Provider News".

The Notification of Pregnancy form was created by DHH in conjunction with Bayou Health to be completed by all health care providers delivering services to Louisiana Medicaid recipients enrolled in any one of the five Bayou Health Plans. Providers should use this form to notify Bayou Health of newly pregnant women being cared for by the provider. This form supersedes any previous NOP forms which are now considered null and void and should not be used to inform Bayou Health of new pregnancies.
