Participant Preliminary Certification Webpage
Thank you for your interest in becoming a participant of the Commodity Supplemental Food Program (CSFP).
The CSFP program works to improve Louisiana's health outcomes by offering nutritious USDA commodity foods to eligible seniors who are at least 60 years of age.
How Do I Qualify for CSFP?
To participate in the CSFP program, applicants must reside in the state of Louisiana and meet CSFP income eligibility requirements.
CSFP uses the U.S. Department of Health and Human Services Federal Poverty Guidelines to determine income eligibility.

How Can I Apply for CSFP?
If you think that you might qualify for the CSFP Program and would like to apply, please follow these steps.
- Click here to access, fill out, and print the CSFP Preliminary Certification Form.
- Mail the completed application to the following address:

I've Sent in My Form. What Now?
Once your CSFP Preliminary Certification Form has been received, the Food for Families organization will review your information to determine your eligibility.
You will be notified within 10 business days of receipt of your completed form if you are eligible for the program.
If you are eligible, you will also receive information on how to reach a local CSFP distribution center to complete your certification.
You will need to bring additional information with you to finish signing up for CSFP. Click here to find out what additional information you will need.
Please note that placement on the program is dependent on availability of caseload space or slots. When slots are unavailable, you may be placed on a waiting list and later certified as slots become available on a first-come, first-served basis. You will be notified within 10 business days of receipt of your completed form if you are to be put on a waiting list for the program.
