New Health Plan Information for Medicaid Members

Healthy Louisiana now has six health plan options. A health plan is the group of doctors, hospitals and other providers who work together to help you get the health care services you need.

These six health plans include:

• Aetna Better Health
• AmeriHealth Caritas
• Healthy Blue
• Humana Healthy Horizons in Louisiana
• Louisiana Healthcare Connections
• UnitedHealthcare Community Plan

Your health plan may change

Before the January 1, 2023 launch, most Medicaid members were auto-assigned a health plan. In this auto-assignment, some members were assigned to a different  health plan while some stayed in the same plan.

If you and your family want to change your health or dental plans, you can do that any time from November 8, 2022 to March 31, 2023 by 6 p.m. You do not need a reason to change health or dental plans between November 8, 2022 and March 31, 2023 by 6 p.m.

You can change your health or dental plan in 1 of these 5 ways:

1. Online: Log into your account or create one at MyPlan.healthy.la.gov/enroll
2. Mobile app: Download the Healthy Louisiana mobile app for free
3. Phone: Call us at 1-855-229-6848 (TTY: 1-855-526-3346)
4. Fax: Send the enrollment form that came with your letter to 1-888-858-3875
5. Mail: Send the enrollment form that came with your letter to PO Box 1097, Atlanta, GA 30301-9913
