Request for Renewal for a 1915(c) HCBS Waiver: Draft (LA.006.04.00)
The Department of Health (LDH) intends to submit the Children’s Choice Waiver (CC) Renewal to Centers for Medicare & Medicaid Services (CMS). Implementation of the provisions of this proposed waiver renewal is contingent upon CMS approval.
