Metairie, La - The Louisiana Department of Health and Hospitals (DHH) will join with the Louisiana Hospital Association and eQHealth to host the Louisiana Care Transitions Summit Thursday, April 9th, from 8:30 a.m. to 3 p.m. The goal of the summit is improving care transitions related to substance use disorders, behavioral health and chronic disease. The event will focus on promising practices for creating more effective transitions in care for Medicaid recipients.
Leaders from the fields of care management, discharge planning, social work, nursing and emergency department leadership, home health agencies, nursing homes and community organizations will be in attendance.
