The Louisiana Department of Health (LDH), STD/HIV Program, is issuing a Request for Proposals (RFP) for qualified proposers to provide Pharmacy Benefits Management services through a comprehensive network of pharmacies, to eligible clients residing in all 64 parishes in Louisiana.
The STD/HIV Program is responsible for coordinating the state’s response to the STD/HIV epidemics including the management of the Ryan White Part B funded Louisiana AIDS Drug Assistance Program. The RFP is available below and at http://wwwcfprd.doa.louisiana.gov/osp/lapac/pubMain.cfm
All proposals must be received at the below physical address by 4 p.m. CT, on Friday, Oct. 19, 2018. Proposals received after this time will not be eligible for consideration. Proposer shall submit one (1) original hard copy (the Certification Statement must have original signature signed in ink), five (5) duplicate hard copies, and one (1) electronic copy (CD or USB flash drive) of the entire proposal. Proposer shall also submit one (1) electronic copy (CD or USB flash drive) of its Redacted Proposal, if applicable. All electronic copies must be searchable. No facsimile or emailed proposals will be accepted. The cost proposal and financial statements shall be submitted separately from the technical proposal; however, for mailing purposes, all packages may be shipped in one container.
