Secure .gov websites use HTTPS LDH is moving forward with a plan to distribute food benefits on EBT cards. If the USDA, who administers the SNAP program, is prepared and ready to move money into states’ accounts, we will use federal dollars to fund SNAP recipients’ accounts. If they are not ready, LDH will proceed with our state-funded program to provide food assistance to the elderly, the disabled, and households with children through their existing EBT cards.

How can I get my vaccination records?
There are several options for requesting vaccination records in Louisiana:
MyIR
Register for MyIRMobile Website. Signing up for LA MyIR provides Louisiana residents the ability to access, review, and print your official vaccination records without requesting them from a doctor. For further assistance with your account, please submit a request to Consumer Support Help Desk.
Ask a Provider
Ask your healthcare provider for a copy. Louisiana has had an Immunization Information System (IIS) called LINKS since 2001. All vaccinators in Louisiana are required to enter vaccines in the LINKS database.
Ask a Parish Health Unit
Ask your local Parish Health Unit (PHU) for a copy.
