Forensic Supervise Transitional Residential & Aftercare Facilities (FSTRA)

FSTRA facilities are licensed by the Louisiana Department of Health, Health Standards Section (HSS).
Definition:
R.S. 28:32
(2) "Forensic clients" means persons transitioned from a forensic facility established pursuant to R.S. 28:25.1(A) or (B).
(3) "Forensic supervised transitional residential and aftercare facility" means an agency, business, institution, society, corporation, person or persons, or any other group, licensed by the department to provide supervised transitional residential and aftercare services to forensic clients including persons in court-ordered conditional release status.
A forensic supervised transitional residential and aftercare facility shall provide clients referred by forensic facilities or under court-ordered forensic conditional release with individualized services to develop daily living skills and to prepare for vocational adjustment and reentry into the community.
Initial Licensing
The Louisiana Department of Health, Health Standards Section, issues Forensic Supervised Transitional Residential and Aftercare Facilities licenses. Once a license has been issued, it shall be valid until its expiration date unless otherwise revoked, suspended, or terminated.
Required Documentation
- License Application Checklist
- A license application approved by the Health Standards Section must be completed.
- Applicable licensing fees ($250). There has been a change in the Payment Procedure. Please follow the link for instructions. This fee is not refundable in the event the facility fails to meet the licensing requirements and HSS determines that a license could not be issued.
- A Letter of Intent must accompany each licensing application describing the DBA name of the facility to be licensed, the geographical address of the ASC requesting to be licensed, the anticipated date of completion of the ASC or the anticipated date by which the facility would be ready for licensing, and the types of surgical specialties to be provided in the ASC.
- Architectural plan approval letter from Plan Review
- Small floor plan sketch to identify all areas of the FSTRA as to its use.
- Office of State Fire Marshal certificate for occupancy.
- Office of Public Health certificate for occupancy.
- Notification to Health Standards Section that application has been filed with the La. Board of Pharmacy for the FSTRA’s Controlled Dangerous Substance (CDS) license.
An on-site announced licensing survey will only be scheduled and conducted by HSS after the completion of Items 1-9.
Renewals
- A renewal letter will be mailed to the provider from the Health Standards Section approximately 75 days in advance of the license expiration date.
- Items that shall be submitted to the Health Standards Section for a license renewal:
- Licensing application
- Licensing fee: Payment Procedure
- Copy of the current Fire Marshall Inspection
- Copy of the current Health Inspection Report
- Proof of Financial Viability
- Proof of $100,000 Line of Credit
- Proof of general and professional liability insurance of at least $300,000
- Proof of Worker's Compensation Insurance
- Once all requirements have been met, a new license will be mailed to the provider on the 20th or 21st of the month before the current license expires.
