EIDR Medicaid Claims Requests

EIDR Claims Reports for Long Term Care Facilities such as Nursing Homes and Adult Day Health Care Centers, and Hospitals are to be ordered through the Rate and Audit Review section. EIDR reports are available to the public in order to assist providers with their Cost Reports. This report costs 1/10^th of a cent per claim line with a minimum fee of $65.00 and will be delivered electronically via an Excel spreadsheet. Additional charges will apply if requesting overnight shipping of electronic copy.

For Long Term Care Facilities, once the request information is completed on the form, it must be emailed to Mary J. Mason at Mary.Mason@la.gov . 

For Hospitals, once the request for information is completed on the form, it must be emailed to Christine Sullivan at Christine.Sullivan@la.gov and Sonya Webb-Forbes at Sonya.Webb-Forbes@la.gov .
