Assessment & Care Planning Trainings Schedule
Virtual MDS-HC and Care Planning Training
MDS-HC assessment and Care Planning trainings are offered ONLY to Louisiana Support Coordinators and Assessors who currently work for a Support Coordination Agency, Program of All-Inclusive Care for the Elderly (PACE), or Long-Term Access Contractor that provide services to individuals receiving Community Choices Waiver (CCW), Adult Day Health Care (ADHC) Waiver, PACE, and/or Long-Term Personal Care Services (LT-PCS).
Only virtual certification classes are being offered by the Office of Aging and Adult Services (OAAS) at this time and class sizes are limited. MDS-HC assessment training consists of two days of virtual classroom instruction, successful completion of a comprehensive online exam, and the completion of one virtual field assessment approved by an OAAS trainer. Care Planning training consists of two days of virtual classroom instruction, the completion of one Plan of Care approved by an OAAS trainer, and successful completion of a comprehensive online exam. Care Planning training is required for Support Coordinators/Supervisors only.
Once an assessor/trainee has completed the required training, he/she will receive an MDS-HC assessment and, if applicable, Care Planning certification which is valid for a period of three years through the Louisiana Department of Health - Office of Aging and Adult Services.
New support coordination agencies will be informed through the enrollment process by OAAS when to register for training.
