Overview:

Project for Assistance in Transition from Homelessness (PATH) is a program to provide behavioral health services to individuals who are homeless or at risk for homelessness.

PATH programs provide:

  • Community outreach and in reach
  • Assessments
  • Case management
  • Drop-in-center services for use as an address and telephone communication site, and to provide referral and linkage to mainstream community services
  • Transportation as needed to primary health and behavioral health services
  • Behavioral health counseling
  • SSI-SSDI Outreach, Access and Recovery (SOAR)
  • Discharge planning with hospitals
  • Housing services, including planning and technical assistance in applying for housing, security deposits, and one-time rental payments to prevent eviction
  • Supportive services including life-skills training, job-training, medication monitoring, and linkages to community resources, goods and/or services needed to improve community living and address adult relapse prevention
  • Wrap-around services to homeless adults with co-occurring disorders
