How can I check the status of my FNR application?

FNR decisions will not be given over the phone. You will receive a certified letter from the FNR Program Committee once a decision has been made about your application.

The FNR Program Manager will only notify you via email or telephone call if your application is incomplete.

The FNR Rule (LAC 48:1 Chapter 125) states:

The review period will be no more than 60 days,
except as noted in the case of issuance of a request for
proposals (RFP). The review period begins on the first day
after the date of receipt of the application, or, in the case of
issuance of an RFP, on the first day after the period specified
in the RFP.
1. A longer review period will be permitted only when
initiated by the Facility Need Review Program. A maximum
of 30 days will be allowed for an extension, except as
otherwise noted for the issuance of a RFP.

Can you provide me with a checklist of what I need to submit for Facility Need Review?

  2. Send  your FNR payment and the HSS Payment Transmittal Form to designated mailing address
  3. Send your FNR applications and documentation to the designated mailing address

Do I have to list a geographic address on my Facility Need Review application?

No, but you must list the service area (based on the FNR Rule: LAC 48:1 Chapter 125) for which the facility or proposed facility is or will be located.

Do I need a certificate of need (CON) to open a licensed facility?

Effective July 6, 1990, Pursuant to La. RS 40:2116,The Louisiana Department of Health, in accordance with the Administrative Procedure Act, has established a facility need review process consistent with the regulations for licensure for facilities and for Title XIX of the Social Security Act. The Louisiana Department of Health does not issue nor has a requirement for certificate of need (CON) for licensed health care entities.
