How to Apply
Complete an Application

Complete Your Interview
After you submit your application, you will receive an appointment letter within 10 days from LDH with the date and time of your interview.
If a face-to-face interview is requested, you may schedule an appointment with any LDH parish office.
Complete an Application
Download and complete a print application.

Applications can be printed and completed at an office nearest to an applicant.

Applying by phone takes approximately 20 to 30 minutes to complete. After you complete your application by phone, a physical copy will be mailed to the address you provide.

When Is My SNAP Redetermination Due?

Do I Need to Report This Change?

How do I report a change?
You can report changes three ways:
- Log in to your CAFE account,
- Call us at 888-524-3578 Monday through Friday 7:30 a.m. to 6 p.m., or
- Visit your local LDH office in person.
When do I have to report a change?
Changes must be reported by the 10th of the month following the month in which the change occurs.
How do I know if my income is above 130% of the Federal Poverty Limit?
The Federal Poverty Limit changes annually. For a household of one, the 130% FPL limit is $1,696 per month for gross income. For a household of three, it is $2,888 per month. If you're not sure if your income is over the limit, call us at 888-524-3578.
What is a redetermination and when do I need to do it?
Redeterminations are the process of renewing your SNAP benefits. You must complete a redetermination at the end of your benefit certification period (typically 12 months) even if nothing in your household has changed. Missing your redetermination deadline will result in your benefits ending.
Who counts as part of my SNAP household?
Generally, your SNAP household includes everyone who lives together and buys and prepares food together. If you have any questions about who should be included in your household, please call us at 888-524-3578 or email us at [email protected].
