Food Safety Certification Provider List

List of LDH Approved Providers who teach the 8 hour Food Safety Manager Course and proctor the ANSI Approved Exam for the Food Safety Manager Certification (Prerequisite requirement for "gold seal" LDH Food Safety Manager Certificate). Certificates not accepted by our office: Food Handler Certificate, Food Safety Manager Certificates valid only in other states such as Texas, Certificate of Completion for Food Safety Manager Course only.

 

Paper Application for LDH Food Safety Manager Certificate

Also requires of a copy of the Food Safety Manager Certification from the testing company for new certificates, and company check or money order payment (Cash, temporary checks, and payments with incomplete/incorrect information are not accepted).
