Nursing Home Provider Information

Submitting Reports when the provider does not have SIMS access:

• Until a provider gains SIMS access, any reports due (initials and finals) must be faxed with a company letterhead/fax cover page to (225) 342-5073 Attention: Wendy Venable/Shari Price and/or emailed with a company letterhead/coverpage VIA SECURED EMAIL to wendy.venable@la.gov and shari.price@la.gov within the regulatory required timeframes. Once you have resolved your access issue, you will need to enter any faxed/emailed reports into the SIMS system.
