- Submit a SIMS Request for the following:
- Having issues logging in
- Forgotten username and/or password
- Reactivate/Deactivate account
- For expired passwords or to change passwords.

Nursing Home Provider Information
Submitting Reports when the provider does not have SIMS access:
- Until a provider gains SIMS access, any reports due (initials and finals) must be faxed with a company letterhead/fax cover page to (225) 342-5073 Attention: Shari Price/John Germany and/or emailed with a company letterhead/coverpage VIA SECURED EMAIL to [email protected] and [email protected] within the regulatory required timeframes. Once you have resolved your access issue, you will need to enter any faxed/emailed reports into the SIMS system.
