Therapeutic Group Homes
Therapeutic Group Homes (TGH) are licensed by the Louisiana Department of Health, Health Standards Section.
A TGH is defined as a facility that provides community-based residential services in a home-like setting of no greater than 10 beds under the supervision and oversight of a psychiatrist or psychologist. TGHs provide a structured living environment 24 hours per day, seven days per week.
The purpose of a TGH is to provide community-based services in a homelike environment to clients under the age of 21 who are determined to need psychiatric or psychological services.
Each TGH shall deliver an array of clinical treatment and related services, including psychiatric supports, integration with community resources, and skill building taught within the context of a safe home-like setting under the supervision of a professional staff person. The goal of a therapeutic group home is to maintain the client's connections to their community, yet receive and participate in a more intensive level of treatment in which the client lives safely in a 24-hour setting.
The TGH application packet shall include the TGH application form and all additional documentation required by the TGH regulations.

NOTE: It is the responsibility of the pursuant TGH applicant to obtain all inspections and approvals by local agencies including local Zoning Boards and ordinances.
Initial Licensure Requirements
Initial Licensing Application Packet shall include:
- completed license application form
- non-refundable licensing fee: the Payment Procedure has changed please follow the link for instructions.
- copy of approval letter of the architectural facility plans for the TGH from Office of State Fire Marshal
- copy of the health inspection report with approval of occupancy from the Office of Public Health
- copy of statewide criminal background checks, including sex offender registry status, on all individual owners with a 5% or more ownership interest in the TGH, and on all managing employees
- proof of financial viability, comprised of the following:
- a. line of credit issued from a federally insured, licensed lending institution in the amount of at least $50,000.00
- b. general and professional liability insurance of at least $300,000.00; and
- c. worker's compensation insurance;
- an organizational chart and names, including position titles, of key administrative personnel and governing body; and
- a floor sketch or drawing of the premises to be licensed
- a letter of intent as to the types of services or specializations that will be provided by the TGH (i.e. sexually offending behaviors);

Renewal Requirements
The TGH must submit a completed license renewal application packet to the department at least 30 days prior to the expiration of the current license. Failure to submit a completed license renewal application packet prior to the expiration of the current license will result in the voluntary non-renewal of the TGH license.
The license renewal application packet shall include:
- Application form
- Non-refundable license renewal fee: Payment Procedures
- Current State Fire Marshal report
- Current Office of Public Health inspection report
- Any other·required documentation
- Proof of financial viability, comprised of the following:
- A line of credit issued from a federally insured, licensed lending institution in the amount of $50,000;
- General and professional liability insurance in the amount of at least $300,000
- Worker's compensation insurance
The department may perform on-site surveys and inspections upon annual renewal of a license.
The renewal of a license does not in any manner affect any sanction, civil monetary penalty or other action imposed by the department against the provider.
If an existing licensed TGH provider has been issued a notice of license revocation, suspension or termination, and the provider’s license is due for annual renewal, the department shall deny the license renewal application and shall not issue a renewal license.
Change of Ownership Information
Providers must complete this document when they have a change in their ownership structure. This document would be used for both a change of ownership (CHOW) as defined by state and/or federal regulations, or a change of ownership information (CHOI) that does not meet the state and/or federal regulations CHOW definition.
For Health Standards to make a CHOW/CHOI determination, all providers must submit the following documents:
- *Letter of Intent (including d/b/a and entity name of the previous and the new owner, the effective date of transfer of ownership, address and phone number)
- *A diagram showing the ownership structure “before” and “after” the change
- *Copy of the executed Bill of Sale
- *CHOW/CHOI License Application
- *855A/B approval letter for the following providers: Home health, hospice, hospitals, RHCs, ASCs,
ESRDs, portable x-ray, community mental health and OPT.
Note: If this action is a CHOI, the documents above are the only documents you need to submit, however the Department may, at its discretion, request additional documentation in support of the CHOI. If so, you will be contacted for any of said additional documents. There is no fee for a CHOI.
If this action is a CHOW, the following documents are also needed:

The fee for a CHOW is usually the same as a license renewal unless the facility is making additional changes. For the providers completing an acquisition/merger, please contact the program desk for assistance.
