LDH is moving forward with a plan to distribute food benefits on EBT cards tomorrow. If the USDA, who administers the SNAP program, is prepared and ready to move money into states’ accounts this evening, we will use federal dollars to fund SNAP recipients’ accounts. If they are not ready, LDH will proceed with our state-funded program to provide food assistance to the elderly, the disabled, and households with children through their existing EBT cards.
December 23, 2014 RFP for Forensic Supervised Transitional Residential and Aftercare Program
Baton Rouge, Louisiana, - The purpose of this RFP is to solicit proposals from qualified proposers to provide a Forensic Supervised Transitional Residential Aftercare (FSTRA) Program with an array of services to conditionally released, and/or other selected forensic clients of ELMHS. Contractor shall secure supervised living arrangements in Louisiana’s Greater New Orleans Metropolitan Area, for approximately 22 – 28 clients with forensic involvement; of which 6 may be female clients. Living arrangements must provide, at a minimum, assistance in development of daily living skills and pre-vocational adjustment in preparation for step-down to a less secure setting one year after admission and independent living eighteen months after admission. The resident’s care shall be guided by individualized, person-centered service plan which is a part of the conditions of court-ordered discharges or diversion from hospital or jail setting. The ultimate goal for the resident shall be successful completion of the requirements of conditional release and transition to the community of the resident’s choice, as appropriate. Cooperation/communication with Community Forensic Services (CFS) and the District Forensic Coordinators (DFC) must be an integral part of the client’s treatment.
