Preadmission Screening and Resident Review
Preadmission Screening and Resident Review (PASRR) is a federal requirement to help ensure that individuals are not inappropriately placed in nursing homes for long-term care. PASRR requires that:
- All applicants to a Medicaid-certified nursing facility should be evaluated for serious mental illness (SMI) and/or intellectual disability (ID);
- All applicants will be offered the most appropriate setting for their needs (in the community, a nursing facility, or acute care settings); and
- Nursing facilities shall provide all applicants the services they need in those settings.
Federal PASRR regulations require that all applicants to Medicaid-certified nursing facilities be given a preliminary assessment to determine whether they might have SMI or ID. This is called a Level I screen. Learn more about Louisiana’s Level I process through the Office of Aging and Adult Services (OAAS).
The Level I screen identifies individuals suspected of having SMI or ID. These individuals are then referred for a comprehensive evaluation through the PASRR Level II process. Due to the nature of the PASRR Level II evaluation, specific documentation and a comprehensive review of documentation are required. The results of the PASRR Level II evaluation provide a determination of the individual’s needs, determination of the appropriate setting, and a set of recommendations for services to inform the individual's plan of care. These recommendations will include specialized behavioral health services that may be rendered in the nursing facility.
The Office of Behavioral Health's PASRR Program is the Level II PASRR authority for those individuals suspected of having a mental illness who wish to reside in a nursing home. PASRR is an important tool for states to use in compliance with the Americans with Disabilities Act (ADA) and the Supreme Court decision Olmstead vs L.C. (1999), which allows for the right of individuals to receive services in the most integrated setting appropriate to their needs.
