Transforming Maternal Health (TMaH) in Louisiana
Louisiana Receives $17 Million Transforming Maternal Health (TMaH) Grant
The Centers for Medicare and Medicaid Services (CMS) has awarded Louisiana the Transforming Maternal Health (TMaH) Model grant of up to $17 million over the next 10 years. This initiative aims to improve maternal and infant health outcomes, reduce disparities, and increase access to high-quality, coordinated care.
This grant will fund the following proposed activities:
- Conducting an Environmental Scan: LDH will begin with a comprehensive assessment of the maternal health landscape to identify gaps, understand community needs, and strategically prioritize resources.
- Data Infrastructure Enhancement: Improve systems to better meet stakeholders’ needs and address care gaps.
- Home Monitoring Expansion: Increase access to home monitoring for diabetes and hypertension.
- Increased Mental Health and Substance Use Disorder Support: Expand risk assessment, screening, referrals, and follow-up for perinatal depression, anxiety, substance use disorder, and health-related social needs.
- Support for Midwifery and Birth Centers: Address regulatory barriers and foster a supportive environment through the Community Birth Initiative.
During model years 1-3, CMS will provide technical assistance to ensure that proposed activities align with the mission and objectives of the model. As part of this collaborative process, current initiatives may be refined to improve desired outcomes.
