Act 769: Pediatric Fentanyl Event Reporting Form

Act 769 of the 2024 Legislative Session requires that hospitals treating minors for suspected opioid overdoses include fentanyl testing in the minor's drug screening. Positive results are to be reported to the Louisiana Department of Health, Office of Public Health.

  • No patient identifying information will be collected in compliance with Act 769. 
  • Hospitals may report data as it occurs, weekly, monthly, or quarterly. 
  • Data may only be submitted using the form below. 
  • Each positive test must be submitted individually. 

Completing this form is not intended to substitute for the mandatory reporting requirements established Louisiana Children's Code Articles 609 and 610. Physicians are mandated to make a report of suspected prenatal neglect to the Child Protection Hotline by calling 1-855-4LA-KIDS (1-855-452-5437). Please follow the protocols issued by the Department of Children and Family Services for mandatory reporting, which includes reporting of substance exposed newborns.
