
OAAS Home and Community Based-Services
The Office of Aging and Adult Services (OAAS) administers home and community-based (HCBS) long-term care services through various waiver and state plan programs for older adults or have adult onset disabilities, assisting them with remaining in their homes and in the community. The intent of these HCBS programs is to provide services and supports that are not otherwise available and that assist an individual to remain in or return to the community.
HCBS programs do not individually or in combination provide 24-hour-per-day supports. This means that in many cases, it may be necessary for individuals to use other paid and unpaid supports at their disposal.
Medicaid Services
Community Choices Waiver:
The Community Choices Waiver (CCW) is a home and community-based service that serves older adults and persons with adult onset disabilities as an alternative to nursing facility care. Based on a standardized assessment, each recipient is provided a budget based on acuity to create an individual service package. CCW contains a wide array of services, including support coordination, nursing and skilled therapy assessments and services, in-home monitoring systems, home modifications and assistive technologies, personal care, home-delivered meals, monitored in-home caregiving, and caregiver respite.
Adult Day Health Care Waiver:
The Adult Day Health Care (ADHC) Waiver is a home and community-based service delivered in an adult day health care center that provides supervised care to adults in a supportive and safe setting during part of a day. Services provided by staff at licensed ADHC facilities include personal care assistance, health education, health screening, medication management, and others.
Long Term-Personal Care Services:
The Long Term-Personal Care Services (LT-PCS) program provides help with activities of daily living for people who qualify for assistance under the program guidelines. In addition to assistance with activities of daily living, this program also provides assistance with instrumental activities of daily living such as preparing meals, light housekeeping and medication reminders. The LT-PCS program does not provide any type of medical or skilled care nor does it provide assistance 24 hours a day.
Program of All-Inclusive Care for the Elderly:
This Program of All-Inclusive Care for the Elderly (PACE) coordinates and provides all of the health care services that older people need, including check-ups, home health and long-term care. The emphasis is on enabling individuals aged 55 or older to remain in their communities while enhancing their quality of life. An interdisciplinary team performs assessments and develops an individualized plan of care for every PACE participant. At this time, the PACE program is only available for individuals in certain parishes of Louisiana.

Other Community Services
Permanent Supportive Housing:
The Permanent Supportive Housing (PSH) program helps people with the most severe and complex disabilities live successfully in the community by combining individualized community-based services with affordable rental housing.

Louisiana's Traumatic Brain and Spinal Cord Injury Trust Fund Program:
The Traumatic Brain and Spinal Cord Injury (TBSCI) Trust Fund program provides services in a flexible, individualized manner to Louisiana citizens with traumatic brain or spinal cord injuries. The program enables individuals to return to a reasonable level of functioning and independent living in their communities.

State Personal Assistance Services Program:
The State Personal Assistance Services (SPAS) program provides personal assistance services to adults with significant disabilities. This program provides these services so that the individuals can continue to live inthe community and avoid institutional care. The SPAS program is funded through the Louisiana Department of Health Office of Aging and Adult Services.
