Maternal Wellness Center Registration

In accordance with Act No. 437 of the 2023 Regular Legislative Session and applicable to income taxable periods beginning on or after January 1, 2025, donations made to eligible maternal wellness centers (MWCs) may be credited against the state income tax for the taxable year in which the donation is made. In addition to meeting other criteria, maternal wellness centers must be registered with the Louisiana Department of Health to be considered an eligible maternal wellness center.

MWCs should complete the steps outlined below to register:

  • Complete the contact information form and submit online.
  • Download and complete the affidavit form.
  • After filling out the affidavit form in its entirety, the form will need to be witnessed and notarized by a public notary by an officer of the maternal wellness center
  • Mail the completed and notarized form to LDH for processing to:

 Applicants will be notified of their approved registration via email.

Registered MWCs are required to submit a sworn “renewal” affidavit to LDH for review and approval annually, on or before February 1 of each year to remain on the registry.
