LDH is moving forward with a plan to distribute food benefits on EBT cards tomorrow. If the USDA, who administers the SNAP program, is prepared and ready to move money into states’ accounts this evening, we will use federal dollars to fund SNAP recipients’ accounts. If they are not ready, LDH will proceed with our state-funded program to provide food assistance to the elderly, the disabled, and households with children through their existing EBT cards.
Add or Delete a Service Module
A licensed HCBS provider may request approval from Health Standards to add or delete service modules.
Home and Community-Based Services (HCBS) providers may offer one or more of the following service modules:
- Personal Care Attendant (PCA) *
- Respite Care*- In-Home or Center-Based*
- Supervised Independent Living (SIL) *
- Shared Living Conversion
- Adult Day Care (ADC)
- Family Support
- Substitute Family Care (SFC)
- Supported Employment
- Monitored In Home Care (MIHC) *
- MIHC, PCA, Respite Care and SIL require Facility Need Review approval prior to submitting an application to Health Standards to add one or more of these services. The FNR approval letter must accompany the application submitted to Health Standards along with the additional fees listed below.
- Complete HCBS license application.
- Mark "X" next to "Add a Service or Delete a Service".
- Mark "X" next to service module(s) you are adding or deleting in the section titled "Service Modules".

Fees must be in the form of a certified check, company check, or money order made payable to the Louisiana Department of Health. Fees shall be submitted in accordance with the HSS Payment Procedure.
