State Health Assessment
LDH is ensuring that data on health and health inequities is widely available to Louisianans. To that end, we have published Louisiana’s State Health Assessment (SHA) dashboard.
The purpose of this interactive dashboard is to highlight where inequities exist regarding health outcomes and the social determinants of health in order to inform the development of programs and policies that can reduce health disparities. This information can be used by individuals and communities across the state to advocate and plan for positive changes in their communities and statewide. The data on this dashboard is updated annually.

NEW SHA Report available!
Between June 2021 and April 2022, LDH partnered with the Louisiana Public Health Institute, the Governor’s COVID-19 Health Equity Task Force, and organizations across the state to share the SHA Dashboard with Louisiana residents and to gather feedback to better understand the status of our communities’ health. We reached nearly 6,000 Louisianans via two sets of meetings for each of the state’s nine health regions, a statewide electronic survey, community leader interviews, and public health system representative interviews. The SHA Report, completed in July 2022, complements the data on the SHA Dashboard as it shares the key findings from this community engagement process and describes the process of identifying priority areas for the State Health Improvement Plan. State Health Improvement Plan
Through the SHA community engagement process, community members identified behavioral health, chronic disease, community safety, and maternal and child health as priority areas to be addressed by the State Health Improvement Plan (SHIP). The SHIP Dashboard is currently in development and will be launched later this year (Click here to get notified). The SHIP is intended to spur nontraditional partnerships between local communities, state agency leaders, local health departments, businesses and various stakeholders by identifying priorities, objectives and strategies to address Louisiana’s most pressing health concerns. Both the SHA and the SHIP can be used by individuals and organizations across the state to align their work for positive changes in their communities and statewide.
Both the SHA and the SHIP represent a collaboration and partnership with community members and organizations from across Louisiana. We would love to stay connected with you. Please fill out this form to join our contact list, and click here for ways you can get involved.

Creating A Blueprint For Our Future
LDH’s prior SHA/SHIP, Creating A Blueprint for Our Future, was originally published in 2016. Completion of the SHA/SHIP process is part of the Public Health Accreditation Board’s requirement for LDH to remain a nationally-accredited state health department.
