2024-2028 Louisiana State Health Improvement Plan

The Louisiana State Health Improvement Plan (SHIP) is your roadmap to a healthier state.

Based on community feedback, the SHIP identifies four priority areas to improve health for all Louisianans: behavioral health, chronic disease, community safety, and maternal and child health.

The SHIP promotes coordination by aligning partners to achieve common goals and objectives within these priority areas, resulting in greater impact. By working together, we will advance the health and well-being of Louisiana residents.

LDH is ensuring that data on health and health inequities is widely available to Louisianans. To that end, we have published Louisiana’s State Health Assessment (SHA) dashboard.

The purpose of this interactive dashboard is to highlight where inequities exist regarding health outcomes and factors that influence health in order to inform the development of programs and policies that can reduce health disparities. This information was used to develop the SHIP and can be used by individuals and communities across the state to advocate and plan for positive changes in their communities and statewide. The data on this dashboard is updated annually.


Between June 2021 and April 2022, LDH partnered with the Louisiana Public Health Institute, the Governor’s COVID-19 Health Equity Task Force, and organizations across the state to share the SHA Dashboard with Louisiana residents and to gather feedback to better understand the status of our communities’ health. We reached nearly 6,000 Louisianans via two sets of meetings for each of the state’s nine health regions, a statewide electronic survey, community leader interviews, and public health system representative interviews. The SHA Report, completed in July 2022, complements the data on the SHA Dashboard as it shares the key findings from this community engagement process and describes the process of identifying priority areas for the State Health Improvement Plan.
